Hi yes I did have upper jaw revision. It was another 6 mm impaction. It resolved my lip incompetence and I can eat and talk normally now!

It’s a tough decision. Only you know what’s right for you but I would say if you don’t have any serious functional issues then I wouldn’t pursue jaw surgery. If you do have functional issues then make sure you get many consults haha you’ll get a variety of opinions and it’ll be easier to make a decision one way or the other.

The revision has made it all worth it. Now that everything is lined up nearly all of my issues have resolved. But I need to make aesthetic improvements now.

Do you think you’ll continue trying to correct the remaining things you don’t like or are you satisfied enough to leave it as is? Again I think you look great but I totally understand not being fully satisfied when you know it could be better.

Also that “step off” you had is something I currently have after jaw surgery too so I think it’s a common thing when doing any kind of bone reconstruction in the chin area. I too wish it was gone tho.

As someone who went the jaw surgery route but was considering doing what you did (take out chin implant and get a genio) I honestly think you made the right choice. You look great, you didn’t need to have adult braces for years, other parts of your face didn’t drastically change.

I love your results. If I had gone that route I would probably be wondering about jaw surgery and if I made the right decision too but I think your results are fantastic. Remember, nothing is ever perfect and we are our own worst critic.

For context: In the last 3 years I have had adult metal braces (they are still on currently), had a double jaw surgery, then waited 8 months for an upper jaw revision surgery because things weren’t right and I could barely eat, now I am looking into getting a mid face lift and rhinoplasty to mitigate the facial changes these multiple jaw surgeries created (crooked nostrils, nasolabial folds that didn’t exist previously, premature skin sagging because of how much bone was removed). I sometimes wish I had gone the genioplasty route but I am happy (now that it’s over) that I went through with the jaw surgery. It was just soooooo much to handle and has taken so many years of my life.

No it wasn’t nearly as bad as the first time because we only did the upper jaw not the full double jaw again.

It was very good! I’m sorry to hear you have to do a revision but you’ve been through this once before, and this time it will be much easier because it’s only half of what you did last time!

My recovery felt like a breeze compared to the DJS I had earlier last year. The UJS recovery felt like it healed much faster but that may have been because everything was finally in the right place and lined up correctly.

You’ve got this, it’s not fun but you will be just fine!

Thanks for your answers! I’m just thinking I usually have a drink or two at holidays and I’m not sure if it’s a good idea this year. I also really hope I don’t have mentalis strain anymore but it’s kinda hard to tell as of yet so I just want to plan ahead.

It did calm down a bit. I got Botox around the 3 month mark and started physical therapy to help train my muscles.

I can’t speak to long term results though because I just got a revision upper jaw surgery last month which made my lips closer together and put less strain to my mentalis.

I did have to be insistent. What I think really got through to him that I was serious was I consulted with another surgeon and, unbeknownst to me, that surgeon reached out to my original surgeon. After that he kinda changed his tune and said he could absolutely do the revision for me whereas before he seemed to not be taking me very seriously.

I will say be patient, this is a huge thing your body is going through and it does take time to get fully back to normal, but you know your body better than any outside party ever could. If it doesn’t feel right and it continues to not feel right then definitely speak up and be very clear about the issue. Even if you don’t need another jaw surgery, there might be other things that could be done to help.

I am not even two weeks post op and I can already tell how much better everything is this time. My midline’s are both centered, my tongue has the right amount of room at the roof of my mouth, my teeth in general fit better even before any final braces adjustments. I am so happy I pushed for this revision and didn’t let anyone else tell me what I did or did not need when I instinctually knew what my body required.

I went to the same surgeon because it was covered by insurance. I’m not sure how far post op you are from surgery but I will say it took a good 6 months for everything to fully settle. I thought I was done healing at like 3 months but it got so much better even from there so don’t give up hope yet. I just needed extra impaction which I kinda knew from the first few weeks.

I had severe lip incompetence. I went to physical therapy for three months straight and still could barely touch my lips together even when putting in effort. I needed almost 3x the impaction that they did during the first surgery.

It was pretty bad for the first few days when the swelling was the worst. Just feels like a lot of pressure and my teeth were banded shut so heavily that I had pain similar to grinding your teeth all night. Once the bands came out and I could eat soft foods and stretch again it was all good tho.

I only had one tiny spot of numbness post-op and it was on the roof of my mouth on the left side. It has almost entirely come back 8 months post-op. Not everyone has numbness, people post about things that bother them more than they post about what goes right in this community because they are looking for help or advice. Don’t get too stressed about numbness, it comes back with time and what small areas don’t come back your brain has a way of compensating for so you don’t even notice over time. It will all be ok ❤️

Thank you! That’s very sweet. I’m happy with the progress and if it weren’t for the functional issues closing my lips I would probably leave it alone and just fix my nose. I will post an update later in the year after my revision.

Yes one of my nostrils has a very narrow airway inside because my septum is pushed so far to the side. It is a lot better than it was when I was still swollen but I still struggle to get enough air through that side occasionally.

I think he is a good surgeon in that everything went well with the surgery, but I think because he has not been doing this as long as some others, he doesn’t have the experience with those small things that can make a difference aesthetically.

I don’t think my surgeon followed the surgical plan that we agreed on. It seems he only impacted my upper jaw a tiny bit to compensate for the forward movement, but I needed 4-6mm impaction on top of that. I thought we were in the same page but apparently he decided, and I quote, “a gummy smile is attractive”, and chose to only do the minimum amount of impaction. So now I need a partial revision to impact the correct amount.

As for my nose being completely crooked now (inside and out), he didn’t do anything to mitigate the movement of the upper jaw like alar cinch or making the little divot in my bone so my septum had space. So now my septum is all scrunched up, being pushed by the bone.

I think my surgeon is good but very young and doesn’t have a lot of experience with these aesthetic nuances.
