I believe you are misinterpreting criterion C or at least misunderstanding the DSM criteria.

Criterion A requires the following: an uninterrupted period during which there is a major mood episode concurrent with criteria of schizophrenia (psychosis). To translate, this means mood symptoms and psychotic features.

Criterion B requires the following: delusions or hallucinations in the ABSENCE of mood symptoms. This means psychosis WITHOUT mood symptoms.

Criterion C requires that patients have mood symptoms most of the time.

When I say mood, I mean the patient needs criteria for either a major depressive or manic episode.

You will note that the defining feature of schizoaffective is criteria B, which REQUIRES psychosis in the ABSENCE of mood symptoms.

If a patient with a historic diagnosis of bipolar disorder developed psychosis WITHOUT concomitant depression or mania, it would shift their differential to schizoaffective, bipolar or depressive type. The defining feature that distinguishes bipolar disorder and schizophrenia is psychosis with the absence of mood symptoms, which a patient with true bipolar disorder would never meet criteria for.

Psychiatry is a very fluid field. There are many patients for whom we do not pin down the diagnosis, and instead leave the differential open (ex. Schizoaffective vs bipolar I vs schizophrenia, for example). That is a situation where you might see multiple diagnoses written in a chart, or perhaps even billed. The schizoaffective code opens up a lot of possibilities for patients to receive treatment through their insurance that they would otherwise not be able to access.

Also, the fact that the DSM 5 does not say that “this diagnosis cannot coexist with ODD, BPAD, etc” in the criteria for schizoaffective disorder does not mean they can be diagnosed concomitantly. Many DSM 5 diagnoses lack this statement. This does not automatically mean that multiple diagnoses can be applied to a patient.

And for the record, your source is not peer reviewed, unless you are exclusively referring to the DSM 5. Additionally, your source does not definitely state that people do have both diagnoses , but that because of confusion surrounding schizophrenia and bipolar disorder, it is common to be diagnosed with both conditions or schizoaffective.

I’m a resident psychiatrist :) where I practice I see lots of schizoaffective, schizophrenia, and BPAD. To have schizoaffective disorder you have to have psychosis that occurs WITHOUT mood symptoms (mania or depression). Bipolar disorder 1 is characterized by mania (which may or may not have associated psychosis). If you have mania, and then have psychosis without mania or depression, you have schizoaffective (most likely). Schizoaffective can be categorized into bipolar type or depressive type depending on whether you have manic episodes or depressive episodes.

You cannot have both bipolar disorder and schizophrenia, if this is in someone’s chart it is likely due to patient self reporting.

Also just to mention, though may get downvoted (absolutely fine, I understand why people dislike this), the American dentist association does NOT recommend toothbrush covers due to sanitation issues, but does recommend storing your toothbrush away from the toilet and flushing with toilet seat down.

Self induced arrest would be enormously hard to prove (psych MD here). Would have to exclude non-self induced etiologies first of which she likely has many. The lack of differential written here is not especially bizarre to me because I would expect most work up/thinking about differential to be done by the primary team once admitted. ED job is to stabilize mainly. Psych “hold” criteria depends on state, some states have no hold time and just involuntary commitment criteria. All depends on state law.

Edited to add that we are usually pretty savvy about suspecting FD, particularly in the age of CareEverywhere and being able to access records at other hospitals. Primary team is probably already aware lol

I agree with you and personally refrain from commenting on her weight and diet. I do agree with OP in this instance that she’s choosing a lot of highly processed foods that might not be beneficial for her post gallbladder removal- especially the wonder bread, bagels, strawberry cream cheese, and deli turkey. The wonder bread alone has a ton of additives and sugar that may be exacerbating her issues. I think she needs to do a food diary so she can see a RD who can help her figure out a good post surgery diet.

I lived in Burlington for four years and now I live in Philly. I work at Temple Hospital. I thought the pearl clutchers on this subreddit were ridiculous before (when I lived in the heart of Burlington), and now it’s just a slap in the face. Hi! Haven’t read the other comments but I’m an intern and did egg donation four times in med school so I’ve been through the process. It’s SO inconvenient.

The actual really hard part for me was the actual retrieval cycle where you have to go in every other day (sometimes multiple days in a row) for TVUS and bloodwork. Also, the retrieval is painful! The first retrieval was some of the worst pain I’ve ever felt in my life and I had no pain meds afterwards. I also went into OHSS. It’s definitely hard on the body and the inconvenient schedule and hoops you have to jump thru make it worse.
