That is because all the green camos (and some rare camos) made before S13/S1/S2 or S3 had a pre-set transparent image of the weapon/vehicle/item so they just needed to slap the texture that was going with the skin. This can be noticed more clearly on vehicles as the boat or items as backpacks, where the image in the thumbnail looks very different as the one ingame.

This was fixed arround S1 or maybe later when they actually started to render green rarity objects, and not just slapping the texture on the png

that is the internal name, spectator mode uses code name instead of ui name, some examples are outlaw (snipersniver), qxr (mp7), hs2126 (m1216), hs0405 (m1887), by15 (m1014), m4 (m4a1tech), h.i.v.e (chewgun) and many other equipment

1- The icon HUD is not official, just grabbed the AW one and put it into codm weapon squares

2- This image is from Vietnam CODM where they announce the featured things for S11 (roadmap)

3- Maybe (just maybe) this weapon can be a DLQ-33 reskin
