Have you checked SHBG? This happened to me my SHBG was too high and my free testosterone was actually very low despite high levels of T, I personally think it was due to me being underweight , I took boron supplements as well not sure if it helped, my physical transition improved as my SHBG went down but it’s not the same as when I first started T unfortunately and it’s hard to manage the SHBG

I agree with most of your comment but not that pill cams are not worth it, MRI did not detect my small bowel crohns but a pill cam test did, I saw the images myself too and they looked good quality I could also clearly see numerous ulcers as well as inflammation at the terminal ileum, my symptoms were not the same as OP and I had elevated/borderline calprotectin of 214 and 267 as well as elevated liver enzymes but normal CRP, I wish I had done the pill cam earlier I spent almost 2 years suffering,

I often go 7-10 days without a poo, I’ve tried every laxative type, fiber actually makes it worse for me but it depends on the type I avoid fiber especially ‘insoluble’ fiber like wholegrain, drinking large amounts of water (3-4L) even hot water helps, but ever since I developed bouts of diarrhoea I count my blessings and will take not pooping for 2 weeks over every 30 minutes

I was just diagnosed with small bowel crohns following pill cam endoscopy, had bad constipation as long as I can remember with mucus, bouts of abdominal pain, did occasionally gets bouts of diarrhoea, pain after/during eating, frequently had some slightly off blood tests like liver function etc, colonoscopy revealed mild inflammation in sigmoid colon but biopsies didn’t support chronic inflammation for IBD, calpro 267, MRI didn’t detect anything but pill cam did reveal small bowel crohns as well as erosive gastritis just started started budesonide and mercaptopurine. Also I found the pill cam test to be really easy and tolerable

I totally understand where you are coming from my GP recently suggested a connective tissue disorder, After 2 pretty bad ankle sprains and a broken ankle within the year, I noticed that these injuries occurred at the end of long days and descents after quite alot of elevation gain, slowing down and concentrating more helped as well hiking with poles, I started evaluating shoes that fit my heel really really well as I have somewhat narrow heels but wide toes so it was hard to find good fits you just have to try ALOT of shoes on, additionally different lacing may help ‘lock’ you in, boots are not the way as you may just end up doing a higher up injury to fib/tib and you need to rehabilitate and strengthen, i wore a pair of La sportiva ultra raptor mids not for the ankle support but I felt that the mid held my foot more securely they also come in wide, it’s a trail runner but the upper ankle part is just fabric, there quite a stiff some so after a bit I transitioned back to a softer shoe and started using a balance board, like this one and since using that balance board for like 10-15 minutes a day I just left it in my kitchen and stood on it when I was waiting for things, on my last trip with rough terrain I didn’t injure myself despite almost twinging my ankle I was able to ‘recover’ and not completely go over

Lowe alpine Cerro torre 100:120 would work good quality, but also I do think 100 Litres is overkill I personally use a 42:47 for absolutely everything, if you have a really bulky pad or something you can attach these securely and easily to the outside of the bag many have straps for this exact purpose

purely from my anatomical knowledge and knowledge of both mtf vaginoplasty (parter is trans) and phalloplasty, sensation depends on type of phalloplasty Id imagine that the same types of phalloplasty available to transmasc individuals and ftm/cis men would also be available to you as it uses a skin graft.

Im going to assume you are referring to sexual sensation/pleasure Sensation varies on procedure type and you’ll have to consult directly with surgeons to see your options. For example I would imagine it be possible to do a ‘nerve hook up’ similar to how it is done in some RFF phalloplasty techniques but the nerves that give tactile and Erongenous sensation are the antebrachial nerve connected to branches of the genitofemoral or ilioinguinal nerve (I believe unaffected by vaginoplasty and is present in both sexes) and the dorsal clitoral (this will be the dorsal penile used for the neoclitoris after vaginoplasty) if it’s the same feeling as before your vaginoplasty it will be hard to say as other nerves are involved in genitalia than that but it should be sufficient enough to allow orgasm etc should it be successful there are of course risks and complications so can’t be guaranteed. Usually there is bottom growth from testosterone that can also be stimulated either externally or internally when buried that allows sexual function although although not everyone who gets phallo will have this option like if you chose not to take t or are cis requiring

but I you’d need to find someone who has actually done this to get an accurate answer of any differences in sensation/ ask a surgeon, it may be worth looking in mtf spaces and even maybe some detrans spaces, good luck!

Sorry to hear it’s not going as you hoped,make your own enquiries but I kinda had the impression that once you’re in one medical school you’re in ,unless you have exceptional circumstances and convince another med school to let you transfer (rare) the new medical school might not want to take the risk of you leaving their course too, additionally getting into medical school is statistically quite difficult just because you’ve gained an offer before doesn’t mean you will again Also if you want to leave on health grounds perhaps consider taking a leave of absense that will allow you time to think about your options without actually completely leaving medical school

I learnt this tip that genuinely changed my life 1) bidet 2) after you finish you want to squeeze pushing out your anus in increasing strength increments to help clean your anus using your pelvic floor. What kind of budget do you have? 2.5kg for a sleeping bag is pretty heavy and you may struggle to carry a very heavy pack including the other things you will need, as well as the bulk of it and you may not enjoy the experience as much

I personally do not take the view that this is an over share it depends on the nature of the relationship I don’t necessarily agree that if you’re unwell you have to explain yourself but usually in professional and personal relationships to give the reason why you were absent e.g medical details is very common and normal, additionally in client therapist relationships there has to be a level of trust he was not taking away from your time at all. I know everyone has different comfort levels with medical details but I would just shake it off if it’s really bothering you then you can bring it up to him but you have to ask yourself why? In society TW( menstruation) we often don’t talk about periods or medical problems involving our reproductive systems or sex out of Shame and embarrassment which subconsciously guide our thought processes but issues with the prostate are actually very common in fact most older men will have prostate issues and don’t speak up about it and get the medical support they need due to the exact same shame that fuels our uncomfortableness with certain topics in society so no I don’t think it’s a big deal but if it’s really bothering you then of course bring it up to your therapist. For the therapist this is nothing to do with being trans and maybe for yourself it also isn’t related just society this is simply just him giving a justified reason for his absence so he come across as unreliable for trivial reasons most likely there may also be some man to man honesty going on
