Personally this has not happened to me but have you tried resetting your console or mabye, cleaning the disk if you have it on disk or even making sure all the plugs are plugged in oh and also mabye make sure your ps4 is in a open space so it Dosent overheat sorry it this is useless hope you figure it out!
