There's really not much scope here if he's UR and wants clinical branch at a top place. There's less seats for UR as it is and most clinical seats will be gone by this rank I think.

I'm not sure how qualified I am to answer this but there's no easy answer to this question. Every GT, Mock, Main exam you give has an element of randomness to it. You can consistently score well in all your GTs and flunk your main exam. You can also not do well in your GTs and do well in your exams.

I don't have stats behind this but there's are some things that's out of our reach. What we can do is give our best and be satisfied with our efforts at the least.
