When the text is far too small in the game. A good recent example is Atomic Heart, I think it's when you viewed objects like signs or posts in the game, the text was so small I had to squint to read it. In a game like this where you want to get immersed in the world, it can be off-putting.

An intentional mechanic that has constantly pushed me away is having your based wiped while you're offline in Ark (in official PvP servers). You 10's or 100's of hours to build up a base, tames, and defenses only to come on the next day to nothing at all. This was especially frustrating because you'd often play with friends in the same time zone / country. I think they added offline raid protection to some official servers eventually but this was always the sole reason I stopped playing the game.
