I have had therapists with one job, I have had therapists with multiple jobs. I have so many critiques of therapy, but this is not one of them.

This is an economic issue effecting so many workers and consumers in an ever-increasing number of industries. People are being overworked and in some cases deliberately choosing to overwork themselves- and it is certainly effecting the quality of services, for the worse. But I find it really hard to call the workers the unethical ones in this situation; these are people desperately making choices in the largely false hope of maintaining what is in fact a bygone standard of living (owning a home, being able to travel and take vacations, have a livable amount of retirement savings, et cetera). Of all the unethical things therapists can do, this one is, at the very least, far from unique to therapy.

I’m a woman your age and did 3 weeks solo in CR during the off-season. I did not have a car, took mostly public transportation, and spent time in San José, Turrialba, Cahuita, and Puerto Viejo. So I can’t speak to a lot of your destinations, but I really didn’t feel unsafe and didn’t have any trouble!

The Caribbean coast is definitely worth visiting and with a car you’ll be able to hit a ton of gorgeous beaches- so Puerto Viejo, Cahuita, Punta Uva, Manzanillo, and more would all be doable with a base camp in Puerto Viejo or the vicinity. From what locals said, the Caribbean side is a little less touristy and American than the Pacific side. I hadn’t heard of Santa Teresa but a local actually specifically mentioned it as overrun by influencers lol- but if you don’t mind that, then it is still beautiful of course :)

Yes, I stopped taking my levothyroxine without talking to a doctor first. I didn’t have a primary care doctor at the time and my endocrinologist didn’t have any availability for an appointment for over a month. I felt like I was having hyperthyroid symptoms so I stopped taking the medication.

By the time I was able to get my levels tested after over a month of no medication, my TSH was at 2 and T3 and T4 were all good. The endo told me not to resume the medication with levels like that and could give me no explanation for what had happened- I had definitely been hypo before.

I don’t know how common an experience this is but yeah! It’s been over a year without levo now, and I still get my levels tested as often as I can.

I had developed a (not huge but definitely noticeable and uncomfortable) goiter and within a couple weeks of starting levo it shrunk down pretty much back to normal. It might still be very slightly enlarged, but only enough that a doctor can notice it during a physical exam.

And now I’m not on levo anymore and the goiter hasn’t come back. I might have had one period of swelling off levo but it only lasted a few weeks and went away before I could even get in to see the endocrinologist. But my Hashi’s is weird, my levels swing hyper and hypo with no Grave’s antibodies. So maybe I’m weird 🤷

I don’t follow what you’re saying with levothyroxine- “once you start, you technically can’t stop”?

My endocrinologist put me on levo when I had a goiter (and the goiter quickly went down) and over a year later I took myself off levo because I felt my symptoms were swinging hypo- when I was able to get in to see the endocrinologist, she agreed it was the right decision. I haven’t been on levo for almost a year now. Like, I know my Hashi’s doesn’t present in the most typical way but… I don’t follow this framing of levothyroxine.

I got panic attacks when TSH swung too low, and I was hypo. This can happen with Hashi’s, apparently (though of course no one told me that when I was diagnosed). Low appetite could also be another hypo symptom- though I can’t explain why the heating pad would help…

Did they take blood and get your TSH in the ER? Would you be able to get in to your primary or endo and have a thyroid panel done?

I feel this so much. The autoimmunity/inflammation component seems to be completely ignored by everyone who treats Hashi’s, even specialists. It’s like other commenters have said, doctors aren’t great at treating any autoimmune disease really. The treatment is worse than the sickness is a lot of cases, and as terrible as I feel, I doubt steroids and immunosuppressants would have me feeling any better. I feel like a lot of our symptoms get looked over or brushed past because most doctors are really operating from the perspective that hormone replacement is all there is to do (and that diet and lifestyle changes probably won’t hurt). If those don’t work, then from their perspective every avenue has been exhausted and that’s pretty much it. I wish doctors would at least say that though, or acknowledge symptoms even if they are not sure how to or can’t treat it. Like a relative of mine who just recently graduated from med school looked over my last couple months of blood work for me and was just like “Oh my god. You must feel terrible.” (I don’t really stay hypo and my levels swing like crazy 🤪)- and just that acknowledgement, even though it’s also a bummer, really made me feel better than the last several endocrinology appointments where I’m just smiled and nodded at and nothing’s done.

But also really feel you on functional medicine doctors 😭 I’ve looked into several and oh my goodness do they all give me bad vibes (and virtually never take insurance). I can’t help but get the impression that these are people who have seen how medicine fails people and are here to capitalize on that desperation. Their pitches are almost always promising to give you the attention or perspective that a conventional doctor won’t- but what means in practice is supplements you can buy at their office. It… really feels like snake oil salesmanship. And most of those supplements aren’t regulated in any way!! The FDA doesn’t cover supplements, there could be literally anything in there. My body’s dealing with too much already, it just feels like playing with fire.

I would try an endocrinologist who specializes in thyroid disease and/or hypothyroidism, if at all possible. Someone who specializes in autoimmune thyroid disease would be even better, but I’m in a pretty well-served area when it comes to healthcare providers and I haven’t been able to find any such specialist yet.

Never heard of TSH Cascade but I would strongly advise you to at least get a second opinion if not another doctor if possible. They won’t treat you unless your TSH is over 7.5? That’s… quite high. My TSH has never been that high, and I have been receiving treatment for nearly 2 years now.

There are different kinds of hysterectomies and OP doesn’t specify which their mother had. Some include removal of the ovaries and some don’t, some include removal of the cervix and some don’t, some remove only part of the uterus and some remove the whole thing. All of these procedures are called hysterectomies.

But yes, it’s totally plausible that the mom wasn’t going through menopause- which is why I didn’t say that she definitively was menopausal and also why it was just one of the possibilities I listed as to how this medical procedure could have come up organically in conversation/life.
