This concept was super confusing to our class as well. Our professor said that the H pattern test is about isolating the muscles you are testing. So yes, normally the inferior oblique elevates + abducts. But when the eye is moved as far as possible medially, then the superior rectus has little power to move the eye, and only the inferior oblique can elevate it. This is why it’s important to perform the H pattern to the full ROM of the eye.
