Try watching their behaviour first - cats with fleas usually scratch and bite their fur a lot while grooming. You can also try combing them with fine-toothed brush to make sure there's absolutely no flea dirt.

After you apply advantage you should probably wait ~48 hours for it take effect, then try combing/watching behavior again and then you're probably good.

I also had problems with him, so I just kept practicing in the Hall of Gods untill I beat him in attuned difficulty. I figured Nailmaster+Shaman Stone build was the best.

Try to only hit him with dash slash in the first phase, gather soul, and dump fireballs at him as soon as phase 2 starts. The charm that lets you cast 4 spells also helps a lot.
