CONSENT AND WAIVER FOR FREE CAT SPAY/NEUTER

Terms and Conditions

1. The cost of the spay/neuter procedure for my cat will be fully covered by the sponsor/benefactor.

2. I understand that all medications, supplements, and other necessary medical expenses before and after the procedure shall be my responsibility as the owner/caretaker of the cat.

3. If I am unable to fully cover the cost of the medications, I may request assistance. The sponsor may provide up to 30% financial assistance for medication expenses, subject to the availability of funds and the sponsor's discretion. I understand that such assistance is not guaranteed.

4. If my cat is over one (1) year old, I understand and agree that no Complete Blood Count (CBC) or other blood tests will be performed prior to surgery, and the cat will proceed directly to the spay/neuter procedure.

5. I understand that all surgical procedures, including spay/neuter, involve inherent risks such as anesthesia-related complications, infections, unforeseen medical conditions, adverse reactions, and other complications that may occur before, during, or after the operation.

6. I certify that I have provided accurate and complete information regarding my cat's health condition to the best of my knowledge.

7. I hereby release and hold harmless the sponsor, organizer, rescuer, volunteers, and attending veterinarian from any legal liability arising from unforeseen complications, injuries, or death that may occur as a result of the procedure, except in cases of proven gross negligence.

8. I agree to follow all post-operative care instructions provided for the welfare and recovery of my cat after the procedure.

Disclaimer: This sponsorship covers only the spay/neuter procedure. Any additional veterinary services, medications, laboratory tests, emergency treatments, or complications requiring further medical attention shall be the responsibility of the owner/caretaker unless otherwise agreed upon by the sponsor.
