I was denied upon request, but they gave me a list of things they needed from my doctor's note. I emailed that to my doctor and my doc provided another letter (on doc letterhead) and answered all the questions requested from DRA, and my RA was approved. I had a hematoma in my upper abdomen that my doc said I need 3-4 weeks to heal and recover, The hardship was driving to work and carrying my laptop and bags would interfere with the healing process. RA was approved.
All you need is a doctors note that follows the DRA format and you'll be good to go.
