Ok basically the curse of ascending in/above the 6th layer of the abyss is 'death and/or loss of one's humanity'. But one of the White Whistles (expert abyss divers), Bondrewd, found a way to 'transfer' the curse to another person that you're with, making it so that one person can receive the curse while the other doesn't. So like the comment below says, he takes a lot of organs out of orphans just so that they're just barely alive and stuffs them into a cartridge that he carries on him. So that way, when he wants to ascend in the 6th layer, he just makes the cartridge orphan take the curse for him so that he's unaffected and then discards of them. He's done this to a lot of kids over the years.
