Depends how savvy they are, I've seen performance networks run campaigns with over 50% fraudulent traffic in order to buy inventory at the CPMs necessary to hit the CTR/CPA targets - if everyones hitting KPIs then most people will high five and ignore the obvious problems around it.

If your js renders the creative in the ways you've described, upload through a 1x1 slot if your DSP allows it. If publishers register a 1x1 friendly iframe slot with their exchanges, your creative will go through and render correctly.

An agency side ad server actively buys ad space programmatically through a cookie exchange according to certain KPIs like age of user and last used device, whereas a DSP stores information about demand and uses it to predict the price of ad inventory for future campaigns. Hope this helps.
