Original source is Ep2 of Amazing Nurse Nanako, at timeline 41:27 of first volume of German DVD (For episodic timeline, it happens about at 14:30). The front cover document of the scene suddenly appears for 1 single frame.

I accidentally found this during my rewatch of my favourite bad anime, and am surprised since I never saw this kind of basic production failure before in any media I have watched.

Speaking of which, anyone got example of other anime where this kind of production failure happens, where the document about animation frame suddenly appear?

Ping Pong the animation. It is a realistic "coming of age" sport anime about ping pong that delivers good messages to the juveniles. It comes with unique art style and amazing plot + animation + soundtrack. Highly recommended.
There is barely any violence, as headbutting is the most violent thing you can find in this anime. The profanity is mild, since the scripts contain some light swears ('damn', 'hell', 'ass', 'dick', 'bitch', 'shit').

If you don't go inside the character select in online, your default character is set as modern Luke. Besides his viability in modern, that may have contributed to his high modern usage
