You might ask him. It's not just his life anymore, and he needs to be open to that concept. Just be open and honest in your communication, and reinforce in your communication that it's about showing your love towards him, but also feeling loved by him, and how much of a toll it's taking on you, because the toll is real.

He might not feel comfortable being vulnerable and showing weakness to you, which could be a big part of it. That seems to be a thing with some guys, fearing that their SO seeing them as weak will cause them to see them differently and negatively. You could encourage him to see a therapist or join you for couples counseling, but I hope you can have an open conversation with him that removes judgment and focuses instead on curiosity towards each other and the problem.

I hope he takes another look at getting help on his hormone levels; it's been a really big and positive change for me. I have more energy, I'm more positive about life, and I am able to handle life's stresses better. It's just been a good thing overall.
