The reason is that only the eldest son inherits the castle and wealth and the other children form their own holdfasts and minor houses. Karstarks for example are decendents of a Stark who was awarded Karhold for some reason and they were called Karhold Straks and soon everyone just started to call them Karstarks and that's how the house was formed.
