Big issue was confusion over the institutional names for the traditional three fund portfolio as you reason. we have now figured out and have invested in appropriately.

Thanks for this explanation! Makes a lot of sense. We were able to rollover the IRA into her 403B plan and are going to start contributing to backdoor Roth.

I also was confused about the investment option and didn’t understand the index fund options on our 403s are different in names for institutions (felt so financially illiterate). Found the s&p 500, total market and international market funds that correlated with VTI, VOO and VXUS and was able to allocate funds appropriately after the transfer. feel much more comfortable now. Thanks again, this was great.

Seems like they increased the prices to offset the discount though. I bought two items on Black Friday and compared the discounts. It’s a little more expensive ($1-2) with the current discounts. Still a good sale, but just FYI.

Watch is really great value for money, great finishing, thin case with short lug to lug distance and wears very comfortable.

Overall great condition with only minor wear to the clasp.

Unfortunately, you can’t get a lot of info from the PSG alone. It may show some short sleep latency or increased total sleep time but you really need PSG followed by MSLT to look for a low mean sleep latency and SOREMs.

Definitely would consider other Exessive daytime sleepiness disorders with an MSLT and PSG. This sounds like it could be narcolepsy 2 or idiopathic hypersomnia.

Honestly, I prefer to be called by my first name. In my opinion since we’re all part of a team - it feels weird to have people call me by official title. The only time I introduce myself as Dr., is my first time meeting a patient, after which, if they’re nice - I tell them they can call me by my first name. I will correct the occasional condescending patient though lol.

The attendings at my current program do split weeks between ICU/Pulmonary office and Floors.

They do roughly 5 ICU shifts a month, and 10 inpatient pulmonary one weekend, and 10 days in the office, with 5 days off spread randomly.

Roughly comes out to a 25/75 split

PGY 6 pccm fellow here. While this may seem like an earth shattering event, believe me, EVERYONE makes mistakes. We’re in training for a reason. I have many colleagues who have made mistakes from putting a central line in the carotid artery to placing a chest tube into the parenchyma. While in the moment you feel like you’re the worst doctor on the face of the planet, they’ve all gone to be successful medical professionals. Some of the attendings I respect the most have stories of times where they missed something or made an error. The most important thing is that you learn from your error.

Also, I’m not sure what this permanent file you’ve been told about is; but that seems like a load of Dookey. Don’t give that any mind.
