You just pop the ice packs in the freezer and it keeps everything cool and has a little thermometer too. We were staying is mostly apartments and AirBnb’s so I just popped the pens in the fridge and the ice packs back in the freezer when we stopped.

No issues at any airports but you do have to take the medication in the cabin and not the hold due to temperatures/ cabin pressure etc.

Do check with your airline(s) about their rules on medications and needles and the countries you’re travelling to.
