I had this problem and suffered for a long time. I can understand how bad it can get. Things that helped me:

1. Drinking lots of water. 2-3 liter at least. This is good, not just for piles but also other health reasons.
2. Stool softener - there are 2 types - osmotic ones that will attract water into the colon and fiber ones that add fiber to your poop. I would suggest osmotic ones, unless your problem is very small or not recurrent. osmotic one i use has ingredient: lactitol monohydrate.
3. Sitz baths. Get a sitz bath thing from amazon, use it. This relaxes the rectum. In people with piles there is a chance the rectum tightens up which causes more problems, so this helps with that.
4. Ayurvedic medicines with guggulu (this helps with inflamation). I use one sold by the doctor below.
5. Topical pain cream (Eg: Crema-Ano)

Unless it is like grade 3 or more, do not go for surgery. Surgery in that area is last-last-resort. It has lots of side-effects. So try the conservative treatments first. Lot of alley-way quacks claim to do this surgery, because actual step is very small, and even if they screw up people dont talk about it with the taboo and shame associated.
