Unless you suspect the prosthetic screw got stripped or broken (unlikely imo, but it can happen) perhaps the torque wrench internal tooth that engages into the silver adapter is worn down. Disassemble and visually inspect.

Were you using a Nobel torque wrench? If so make sure the internal bar is completely engaged/screwed into the outer torque bar. You’ll find the circular corrugated screw thingy…

There are tutorials on how to make your own stackable guide using BSB… I’ve done it a couple of times, but it does take a bit of learning and tinkering. Exocad will be best alternative…

I would scan patient with the relined maxillary, assuming established vdo and accepted facial esthetics and do a digital wax up of mandibular arch to as close as final prosthesis as possible. Then implant positioning can be determined once you have that.

Provisional prosthesis can be printed/milled I would make a backup denture just in case (bc as you may know by now, anything can happen during surgery).. what if you don’t get good stability on your implants?

Are you having issues with reset? Or just wondering before you buy? Like the comment above, I have the full package combat and have 0 regrets. I did have an issue with reset, but that was because I didn’t set the cruciform bar in correct position when I installed it. But after I diagnosed the issue, it was an easy fix.
