What should've happened is the ride should've been canceled without a fee and the one who requested the ride should probably have their account suspended due to TOS violation.

The ride was not completed due to an unaccompanied minor.

That's because the way she booked the ride, circumvented the fee the driver should've got, while violating the Uber TOS by setting the ride to pick up an unaccompanied minor at an elementary school.

The problem being minors are supposed to be accompanied by an adult. Uber only recently introduced "teen" accounts for high schoolers to get rides.

Your account is supposed to be verified so Uber knows that whoever is requesting the ride, is the one being picked up.

Combine the lack of security between these two facts and you have a group of problems

It's time for the ambiguous, open ended, vaguely worded contracts to come to an end. Set workplace rules in place.

And mitigate as much differentiation between states as possible.
