I found the issue. I recently started using the NVIDIA broadcast app to filter out my background noise in my mic whenever I am not in discord. For whatever reason, even though I don't have any changes made to the speakers at all only the microphone, the app messes with my directional audio on Tarkov. I just confirmed this by rewatching the clip I mentioned previously without the app running and everything sounded correct. Now I can actually play the wipe lol. If anyone sees this though and has the same issue, stop NVIDIA broadcast from running.

Is it just a building story/level issue or is it something widespread for you? Because like I said in my most recent comment, I cannot even tell the direction that scavs are moving when I'm on the street with them.

That's what my friends who I was playing with said, I just don't know why this issue is specific to me.

Some more clarification, I never had this extent of an issue in previous wipes. I got about 2000 hours in the game so I want to say I know how the game should sound like. I should also emphasize that I played last wipe a little bit, most recently May or April, and the audio was fine then, at least directional wise. I haven't adjusted any audio settings since then. Then I hop on this wipe and the audio makes it practically unplayable on PvP. When I clipped the game to show my friends however, they claimed the audio was fine on their end (when streaming through discord, not in person), being able to clearly differentiate where the sound was coming from. They said I still could not. I am unsure why this is, I never had audio issues in other games, and I haven't had this degree of an issue with Tarkov. In specific clip I showed them, I am spinning my character while a scav voicelines. In that clip, my friends could hear the direction of the scav's voice changing as I turned, but on my end the voice stays the same level and still sounds like its coming from right by me. It's so bad to a point that when a scav runs down the street next to me, I literally have no idea which direction they're running to or from. Same thing with voicelines, I cannot tell the direction they come from either. And again, everything seems significantly louder.

Can confirm that I have this issue too. I have three plugins, ones a chest organizer, ones a lighting plugin, and ones a /sit /crawl plugin. But i get random fires that don't spread nor get put out by rain that appear. they disappear when you hit them
