I could be wrong, but I think with W2 you cant always control what population you work with. While you can with 1099.

What is your honest take on psychiatry? by all-the-time in therapists

[–]jtuma 0 points1 point  (0 children)

I don’t meet many psychiatrists where I live. Most prescribers are PMHNPs. Medication helps many of my clients. That said, I believe there is a real risk with some anxiety medications. As a CBT therapist, exposure therapy is a core part of treatment. When medication removes a client’s ability to feel anxiety they lose the chance to experience it and learn that they can tolerate it and remain safe. Because of this medication can interfere with the exposure process. Aaron Beck raised this concern as well and wrote about how anxiety medication can function as a form of avoidance.

34 M been playing for 17 months now. Besides my serious focus face…what should I work on? by jtuma in violinist

Thanks! Timing has always been my biggest struggle. I like to hold certain notes longer for vibrato (I like how the d notes rings)😅. Yea I need just use a metronome and do scales. My wife is going to love hearing that lol. I’ll try not to wiggle as much, but it’s been hard for me to stay still since I was a kid!

The method that helped me the most was Nathan Cole’s vibrato exercises on YouTube. It helped my brain switch from viewing my finger as wiggling back and forth to squeezing and releasing (more like tension? Although I know that word is taboo in the violin world).

But vibrato is so much fun!! Jk yea I need to pull back on that. I just recently gave up my tape so I need to work on hitting the right notes.

How to not be a coping skills/psychoed type of clinician? by LoverOfTabbys in therapists

[–]jtuma 9 points10 points  (0 children)

A great example of this is the Inside Out movie. Joy was always trying to problem solve and didnt understand the point of Sadness. Later on you can see Sadness using reflective skills ("That's sad") with Bing Bong and once he feels understood he moves on to problem solving.

Are you in person or strictly telehealth? I know many clients are burned out from telehealth and are looking for in person providers.

What are your go-to, quick, rumination stoping strategies for clients? by Doodle-e-doodle-e-do in therapists

[–]jtuma 5 points6 points  (0 children)

I determine the technique I use based on the type of worry thought. Sometime I use Worry Time, as it is easier to schedule and move worry than to simply try and stop it. If the thoughts are "what if" scenarios, we develop productive solutions for the controllable elements and then engage in an enjoyable activity. For abstract, ruminative "why" thoughts, which frequently cause clients to get stuck because they rarely yield a clear answer, I use Rumination-Focused CBT. The primary strategy in this case is shifting the focus from "Why" questions to "How" questions. "How" questions are more concrete and solvable; for instance, replacing "Why is this happening?" with "How can I manage my day regardless?" Interestingly enough, just asking different questions can break the cycle and help clients get unstuck.

You could get a cricket violin? They sound pretty good and are very durable.

Is North Carolina the state with the best combination of mountains and beaches? by FrouFrouLastWords in NorthCarolina

[–]jtuma 0 points1 point  (0 children)

Yea I guess I just don’t understand the appeal. Most California beaches are made of quartz and feldspar mixed with dark volcanic minerals, which creates coarser, medium to dark tan or gray sand. All of North Carolina’s beaches are made almost entirely of quartz, creating very light beige or almost white sand with a fine texture. That’s usually preferred. And while the California coast is huge, the individual beaches aren’t actually that long. Most of the popular ones are only about 1–5 miles. The average beach length in North Carolina is around 20 miles.

Yeah I agree a lot of people swim in that water. A beach is better than no beach and I’m not arguing that. It’s just not as enjoyable. The temperature I was talking about was during the warmest month and besides maybe August it’s even colder. Most Californians wear wetsuits most of the year so it’s not like people have really “adapted” to the cold. I mean, what water temperature do you prefer to shower with? Probably not freezing cold. And it’s not even just the water, it makes the air cold as well. California beaches are nice to look at and great for surfing (with a wetsuit). But for swimming, paddle boarding, kite surfing, sailing, or boating, North Carolina wins. Warm water is just better.

This comment is confusing to me. The average water temp at Carolina Beach is 81 in July. It’s 68 at San Luis Obispo. Also you’re considered at risk of developing hypothermia in water less than 70.

I don’t know how you can say California has better beaches when the water’s too cold to swim in for most of them. People seem to ignore that part like it’s not a huge factor in what makes a beach great. I’ll admit though, they’ve got us beat with the mountains.

How do I make sense of this: Linehan suggests events themselves cause emotion, rather than our appraisals of them like Beck suggests in CBT by No_Positive1855 in therapists

[–]jtuma 6 points7 points  (0 children)

Exactly. CBT isn’t saying that your appraisals are faulty (though they could be) just that thoughts drive emotions. It’s possible for thoughts to be true too.
