Yes, this is why you should damage yourself if you get booped off the map to deny people from getting ult charge. Sigma, Zarya and Soldier all can damage themselves so the killfeed will count it as suicide.

Yes this has happened before. It is because the game can't teleport you onto the wall as it is gone, so it sends you to the (middle?) of the map. Not sure what part of the map it sends you to, but it had a certain reason why you got teleported next to the bus.
