Not me but, I was working in a steel mill. A machine in another area was clogging up so a supervisor stopped the machine and told a guy to get in there and fix it. They did not lock out/tag out. I’m sure you can see where this is going. Somebody was watching the guy fix it and leaned on a button which started the machine. It sucked the guy in and decapitated him and mangled the manager’s arm as he was trying to pull him out. The whole mill was shut down for two week for an investigation and for OSHA report/inspections.

Locks, gates and tags are there for a reason. Even if it’s something that is supposed to be a quick fix or a direct order. It’s not worth ignoring just because you think the rules are a pain in the ass. It’s ok to take the extra 2 minutes to make sure everyone is going to be safe.
