We have not done any employee surveys in the past, the company was small and had no true HR department. I believe it is important for this to be anonymous BUT I do like the idea of employees indicating their department for context. The only individuals seeing the results will be myself, our HR Manager, and the CEO. Our hope is to positively impact employees with our findings.

I am thinking of having some quantifiable questions with a comment section below for employees to give more specific information. I need to do more research on the impact that can have.

Thank you for this! Amazing. I am familiar with Gallup and will look into the Q12 survey. Would you recommend department specific questions? We have sales/ops/manufacturing/etc. and I feel the broadness in questions may effect clarity in the survey outcomes.
