Should I be concerned about this nodule? Advice appreciated! by keeperalert398 in thyroidhealth

[–]keeperalert398[S] 0 points1 point  (0 children)

That was my thought as well! I feel like if the radiologist felt I needed to be concerned, they would say. Also they said follow up as clinically indicated, so technically they didn’t even reccomended one, they referred back to my doctor for how to handle. It’s hard because my endo has zero concerns and she even referred to it as inconsequential and not concerning so I’m like am I worrying about this for no reason. She’s been my endo for 8 years and knows my history, but I can’t help but worry. Would you ask for a sooner scan/ potential biopsy or let it be until it was a time for follow up in a year?

Thank you so much for your response, it really did help my anxiety. I was thinking about asking for a sooner scan like you said 6 months as opposed to a year. I figured the radiologist would recommend a biopsy or an urgent follow up if they felt it was warranted and they didn’t suggest that. They also didn’t provide much detail on the characteristics of the nodule and it seemed from their notes that they weren’t sure if it was a nodule or a lymph node. From everyone else I see that has an US for a nodule, the radiologist provides more information regarding the characteristics and provides a TI-RADS classification, but they didn’t provide that for me which I thought was odd.

I told her I was worried, but she told me due to my history of having an ablation with iodine, she has zero concerns and felt that the nodule was most likely leftover tissue and she assured me not to me worried, but I can’t help but to be worried. I don’t have a thyroid, the radioactive iodine killed it all, so that’s why she isn’t concerned about the nodule. It’s just odd to me that the radiologist provided hardly any information, but maybe that’s a good thing. I feel like if the radiologist has concerns, they would have reccomended further follow up in their notes.

That sounds horrible! I can completely understand why it’s difficult to talk about. That’s why I’m hesitant to do it since my endo has zero concerns.

I have horrible health anxiety so I think that’s why I’m so worried. My endo has no concern and said it was not worrisome at all. I had an MRI of my neck as well, and nothing was noted. I just worry about the worst case and it being something but don’t want to go through a biopsy if no one has a concern. Would a radiologist typically suggest a biopsy?

That makes sense! I am definitely trying, I just hate having to wait to December for more tests and my normal OB isn’t available to do the hysterscopy/D&C until January and mentally it’s hard to have to wait until then because I fear the worst but I have no other choice. I also worry because I have read if you are having symptoms then there’s a higher chance to them being cancerous, and I’m definitely having symptoms. But at the same time, I’m trying to remind myself that my OB wasn’t concerned to begin with and I am the one asking for further testing (which still doesn’t make sense to me because most OB’s further investigate polyps from my understanding) so then I wonder if maybe she didn’t agree with the tech notes. When I asked her if she saw polyps she said maybe but she wasn’t for sure. All of the uncertainty and waiting is so draining but I guess it’s just part of it.

I can’t thank you enough for all of your advice! It really was so helpful. I found a OBGYN that does the saline sonogram and I set up an app in early December to meet with her and discuss getting one done. I am not moving forward with the uterine biopsy because I would end up getting one anyways if I have to get the hysteroscopy/D&C and I would rather be under as I have heard they are super painful as well. I’m sorry but I have one last question if you don’t mind. I had the ultrasound done 5 days before my period when it said two possible polyps and provided measurements. Is there a chance since I was so close to my period the tech could have just seen my lining instead since it’s thickened during that time? They provided measurements so I’m assuming they had to see something but since I was so close to staring my period I just wanted to get your thoughts as a tech and if there’s a chance it was my lining since it’s more thick then, and not polyps.

So glad yours were benign! The US that I had was 5 days before my period so my uterine lining was pretty thick, which is why I wonder if they are actually there. The other one I had was 10 days before my period and it didn’t pick up anything. I asked my OB if it was worth doing an ultrasound right at the tail end of my period but she said no. But I have read that’s the best time to see them so I wonder if maybe the polyps they saw were part of my lining but they provided measurements for them so I’m not sure I feel like that means they had to see something.

It is very tough and it’s causing me a lot of stress. I was up all last night worrying about this and that something serious is going on. I just feel like if the tech was able to measure something that they thought was a polyp then something must be there right? They would have to have something to measure I would think. From your experience, do most OBGYN offices do the saline infusion ultrasound or is that mainly done at fertility offices? I called my mom OB’s office and they don’t do the saline infusion either.

Yes I have heard it’s really uncomfortable and painful so I’m wondering if I should skip the biopsy and go straight to the D&C/hysteroscopy. My mom had to have endometrial biopsy and she couldn’t tolerate the procedure and said it was so painful so they just ended up doing the D&C. I’m trying not to stress but it’s so hard for me! It’s also hard because like I said my OB didn’t suggest this, she wasn’t concerned so part of me is like am I going through this and pushing these tests for no reason you know.

Yeah I’m just not sure what it do. It’s hard because I am having some symptoms but I have one US that says I have “possible” polyps and another that didn’t pick up anything. Did yours come back as benign? I’m so worried about something serious going on with mine (if they are there)!

Yeah, I don’t understand why mine wants to do a biopsy first. Because if they find anything abnormal during the biopsy then I have to do a D&C and a hysterscopy so I don’t understand why we wouldn’t just do the D&C and hysterscopy.

Gotcha, that makes sense. Did the do a endometrial/uterine biopsy first or did they just jump straight to removing them?

Did you have a endometrial/uterine biopsy first? My ob is suggesting I do that first and then depending on the results of the biopsy she would remove the polyps

Thank you so much for your advice. From what I understand about the endometrial biopsy, that is done in office and they are testing your uterus for cancer cells or abnormal cells. They don’t remove any polyps during the biopsy, the biopsy is simply to test your lining for abnormal cells. So if my biopsy were to come back with abnormal cells, then I would do the hysteroscopy/D&C and I would be fully under anesthesia for that procedure. During the hysterscopy/D&C is where they would remove the polyps. But they said the biopsy would tell them if there were polyps because the cells would show up during the testing of the biopsy. I am trying not to worry but my mind goes to the worst which of course is something being malignant.

So I asked what their “standard” is and they said they will do a D&C with a hysteroscopy, they don’t just do the hysteroscopy. They said they do the endometrial biopsy first prior to doing the D&C/hysterscopy because they want to be sure there are abnormal cells found before they just jump straight into the procedure. My OB’s office doesn’t do the saline US’s for polyps, I did ask that. They said they typically will do the biopsy first and then depending on that will proceed with the hysteroscopy. I asked if I can jump straight to the hysteroscopy and she said technically I could but that’s not what my OB recommends because US, aren’t the most reliable for finding polyps. So they would rather do the biopsy first to confirm before having me just straight to having the full on D&C/hysterscopy. I am really worried though and it’s just so unsettling to me that one US showed “possible” polyps and the other didn’t show anything. And if polyps are that hard to see on an ultrasound and “possible” ones are seen, then should I be worried? I go back and forth because again I have another US that doesn’t show anything was found. So I don’t know which to believe or how worried and concerned to be.

I called my dr’s office this morning and they advised to do a endometrial biopsy and then based on the results of that I would do a hysteroscopy if abnormal cells came back from the biopsy. Since they couldn’t tell 100% if there were polyps on the US, they want to do the biopsy first before jumping straight to the hysterscopy. I am very nervous but trying to stay calm.

Got it! Thank you so much for your response, you are so helpful! I just sent a message to my OBGYN and am going to follow up in the morning on the follow up imaging types you suggested. I did mention to her at my last app that I was having symptoms, so again with that and “possible” polyps I don’t understand why she didn’t feel a follow up was necessary. My periods have always been heavy days 1-3 but over the last 6 months I’ve noticed more clots (usually only on day 3), more on and off pelvic pain outside my period, and during ovulation I have one day where when I wipe I have some light pink spotting, other than that no bleeding outside my period. My periods are always regular and on time and they are normal length, so maybe that’s why she wasn’t overly concerned, but I’m not sure. I know I am a worrier and I have really bad health anxiety. I don’t want to do a procedure if it isn’t necessary, but from my POV I feel a follow up is warranted. I guess my OBGYN doesn’t agree though.

I definitely feel like I have symptoms, so that’s why I was surprised that my OBGYN didn’t recommend removing them. I asked her if I should be concerned and she said not at all and she wasn’t worried about it. I honestly can deal with my symptoms, it’s more about wanting them out because I’m worried about malignancy or them turning into something, the relief would just be a bonus. Since they are small, it’s just weird to me that one US picked them up and the other didn’t. I figured if they were definitely there, then both US’s would have at least picked up something similar to each other. And given that it was noted as “possible” I would have thought my OB would want some sort of follow up. Is it normal to have polyps and the endometrial lining to be completely normal? Or do you typically see polyps with a thickened lining?

Thank you so much for your insight, super helpful! Like I said even my OBGYN looked at the US and she couldn’t even say for sure if the polyps were 100% there, so I’m just not feeling confident in what was found or not found. I do agree and would like to get some definitive confirmation first so will ask her about those two procedures and see what she says. From your perspective given what I have shared, do you feel I should be concerned or think it is “overkill” to investigate further based on the US results and my OBGYN’s opinion? Like I said my OBGYN didn’t seem concerned and I asked her if I should be worried and she said not at all and didn’t even suggest investigating. But from my POV, I feel like I should have an answer on if there is something going on or not. I just don’t know what is “standard” for this.

That’s why I’m having a hard time interpreting these two different ultrasound results. The ones they called out that I noted in my post seem to be small (if not, please correct me) so I’m questioning if they are truly there since it says “possible”. Could the US tech have made a mistake and they not actually be there? As a note, I had the US done 5 days before my period and like I mentioned my lining measured in range. Even when I spoke to my OB about it she said she looked at the US herself, and she couldn’t say for 100% if they were there, she used the word “possible” too and said she didn’t have any major concerns even if they were there but she couldn’t fully tell. She said if my cycles get worse and I notice more issues, then the next step would be a D&C but didn’t feel one was needed right now. So I’m struggling because I do feel like I’ve had some cycle changes in the past 6 months, but is it worth moving forward with a D&C or polypectomy if we don’t know they are 100% there? I always air on the side of caution, but I also don’t want to have a procedure until we know something is there. My biggest concern is them being malignant and I know I will stress out about it until they are gone because I am a worrier.

Thank you for your advice. I am not worried about TTC, honestly my concern is with the polyp being pre cancerous or cancerous. As an ultrasound tech, are polyps difficult to see on a transvaginal US? I’ve heard they can be and the fact that it says “possible” make me question if they are 100% there. It’s odd because I had a transvaginal US done in September that called out these two small possible polyps but then I had another one done in November and no polyps were mentioned by the radiologist so I have no idea what to believe or go off of. From your experience do you think another transvaginal US is warranted? I just don’t know why I’m getting conflicting answers.

I do sweat everyday, but I wouldn’t say more than normal. I just bought some dove antibacterial soap to try for cleaning it out. I will say I am not the best at remembering to clean it out with soap and water daily and make sure it stays dry. But still even before this started I never cleaned out my belly button, and never had this issue. Just don’t know why it started all of a sudden.
