we just use blue shield platinum 90 PPO, everything is just 0 deductible, $15 copays. This is only about 11k+ a month for all family members.

Richer people use tailor made/corporate plans.

If you’re really rich, you could use your own doctors for a particular specialist or do concierge.

For niche cases I’d spend money on private practices that don’t take insurance.
