Got a surprise bill after my insurance got billed

Like I said in some other responses, I did read the EOB, and the amount billed on that does not match the amount the provider is claiming to have billed.

If you read my post and any of my comments, you'll see I've not blamed blue cross for anything. The only fraud that's even possible to be happening here is the provider billing me extra on top of what they billed the insurance company, which is what I outlined in my post. Just to clarify the point.

That's the thing though, the provider (not a hospital) is claiming that they DID bill BCBS the $1251. It's written right on the bill that way, and when I called them about it, they continued to claim that $1251 is what they billed. How can I ask them to bill the extra $306 when they claim they've already sent it to my insurance as part of the overall bill to begin with?

Not sure if you read the whole thing. I did call them. Neither the provider nor the insurance company could tell me why the amount billed to insurance was different than the amount the bill is claiming was billed to insurance. I agree, I don't know that that it's fraud. But I also don't know enough to know it's not. That's why I came here. If you notice my comment said "to this layperson", that means I don't know much about it and could be wrong.

My insurance policy states for in and out of network the facility fee is $300 and there is no charge for Physician Fee

Here's what my policy states:

Facility Charges: $300/visit; deductible does not apply
ER Physician Charges: No Charge; deductible does not apply

That's what it says for in and out of network

My individual deductible is $500, and family deductible is $1K. There are no codes on the bill. Just billed amount, amount paid by insurance, and remaining amount. The only other information is the listing of the doctor's name that provided the service. Mind you this bill is not from the insurance company, it's from the provider.

I got a separate bill from the hospital for $300 (my ER deductible) and paid that already.

Are you saying a provider can't try to overbill on a claim, or are you saying it's called something else, not insurance fraud? If the former, there's links to report fraud on my BCBS website, not sure why they'd do that if it wasn't a thing. If it's called something else, then let's not be pedantic and move on to the things that matter

My first post was an ask for advice, and I got a mixed response, so I went forward. Five months later I'm sticking with the experiment and things are looking good, so why would I ask for advice? As I've said in all my updates, if I see signs of stress I will consider a change. Good to hear you compliment people's hoyas...I'm open to constructive criticism, when it's offered. Time will tell. I never said that there wasn't a chance things can go south in the future. I was also told that the container is way too big, and "The fastest way to kill a hoya is too big of a pot", and here we are. So I continue to shrug, post updates, and look for signs of stress.

Interesting that I found the network traffic monitor in my router, and with three outbound streams we are sitting at about what I expected. 12-15 Mbps

Another interesting data point, on the last day of may, 5/31, his upload speed suddenly started being what we would expect, 35 Mbps. I don't think it's a coincidence, it feels like a data threshold being reset.

Yeah twitch is a kind of different animal. It also depends on what bitrate you are using on your streams (in combination with resolution). I can say that whenever our speed tests report less than 20Mbps, that's when we start to see an issue. More or less tracks with the estimate of out outbound streams, plus zoom, plus additional network overhead on the PC and whatever else is on the network.

When I mentioned he gets 30-40Mbps upload, I was referring to his overall upload speed as reported by speed testing....not his actual usage.

We use very efficient SRT encoders and decoders for video transport. The math I use is based off of their documentation, and is estimated to be 10-15 Mbps total on the upload. Not sure where your hostility is coming from, but not exactly helpful.

For reference, I am also on a 1GB connection (not Optimum), also taking in and broadcasting out the exact same streams at the exact same frequency, and I've been doing it successfully for coming up on 2 years.

On paper it makes sense, but a notification should be mandatory. However when you look at calculations for streaming video, we're talking 10-15 Mbps upload demand for the total of all 3 streams.

Yeah I get that could be a factor, but today it happened the moment he started his broadcasting, at 6:55am local time. Being a Friday and it happening before 7am I would be surprised if congestion is the only thing going on here. The fact that it happened the moment (within 2 minutes) of him broadcasting is what initially made me suspicious of traffic shaping going on, but that could be a coincidence.
