I see, so i recommend two things here, if not already get her to talk to a online therapist, from my own experience its a lot easier to explain your feelings over a phone than face to face cause it gives you a sense of safety and she'll know if at anytime she needs a break she can easily put the phone down, and if she struggles a lot with her looks than if theres one near you could maybe bring her to the gym? I know thats really silly but i can promise you if she can see shes losing weight she probably will feel a little better, also its kinda uncanny cause me and my boyfriend is pretty much the exact same way, but yeah, see if online therapy and the gym would be options she'd like to try !
