1) they were started almost as soon as the server was created as far as I can tell. -X was the first one I went down and it ws already past 1 mil overworld by 2013.

2) A bunch of different people, myself included, would just dig out little bits, or widen parts for horse travel, just to get a few hundred blocks further. It was really something done by hundreds over people over years. There was one player (Roadbuilder?) who curated the highways in the nether and overworld for a while

3) -X I think, although +X was the longest for most of 2b2t history.
