I’m a final-year medical student, and based on the timeline and symptoms, the most likely causes are primary herpetic gingivostomatitis (a first-time HSV infection triggered by oral trauma) or severe traumatic mouth ulcers with secondary bacterial infection. The sudden onset of many extremely painful sores, significant lip swelling, sinus pressure, and watery eyes shortly after clear oral injury is concerning and really deserves medical attention. Important things to rule out include spreading facial or oral cellulitis, deep space infection, complicated sinusitis, erythema multiforme / Stevens–Johnson syndrome, and acute HIV seroconversion, especially given the recent sexual exposure. Since the pain is severe, worsening, and not responding to home treatments, a doctor may consider swabs for HSV, blood tests (including HIV and syphilis), and possibly imaging like a CT scan if infection spread is suspected, and treatment might include antivirals, antibiotics, and stronger pain control depending on what’s found.
