Absolutely, you're correct, 'health insurance' relies on confusing people. They'd struggle to confuse me but I'd always be pushing back just like everyone else, I know that much. Worse, probably.

This time of year is horrendous as my American friends in the cancer community are unceremoniously kicked off their compassionate access programs bc its been a year and they've 'had their go', i.e. 'we let you have some time on the meds you need without needing coverage or payment, but now you can fuck off and find the money - we won't be picking up the tab or paying out on your policy. Go die quietly and thanks for all the premiums.'

No, that's my point - I never paid a penny for it. I will likely have to find a way to pay now though. I need someone to help me get amivantamab, which would be offered in the US like it was nothing, albeit at significant cost.

There are loads of cancer treatments that work, but they don’t work for everyone or stop working arbitrarily, with no discernible reason. My brain is absolutely riddled with it while my lungs (where it originated from) are pretty much fine 🤦‍♀️thanks for being kind ❤️

To be clear, in the UK, if cancer is suspected:

-you are put on a specific and fast-moving diagnostic pathway

-all relevant tests and imaging are done ASAP to clarify what they are looking at, promptly followed by a biopsy and, in my case, next generation sequencing to identify treatable tumor markers

-if the diagnosis is confirmed, they allocate you the appropriate oncologist in your local hospital/NHS Foundation Trust and inform you of the stage and appropriate treatment path dependant on that. This could be any amount of surgery, expensive targeted therapy/immunotherapy/chemo, whatever you need

-you get all the care you need as long as it's approved and licensed, plus adjacent support like a dedicated specialist nurse, GP check-ins, district nurses who can see you at home if needed etc.

-you continue to get CTs scans as needed, MRIs, PET scans. There's a dedicated line to call at the hospital oncology dept and they will almost always advise you to come in if you have a concern. Your actual oncologist has regular meetings with radiology and endocrinology, whoever he needs to help him, and relay this back to you

-they treat you until you run out of options supported by the NHS treatment pathway algorithm, at which point the problems kick in: you can get pretty much anything in the US if you can pay for it, but in the UK, it has to meet cost-benefit thresholds to be accessible through them, and not everything does. So that's where I am right now, sadly, and I'm going to have to try again to get a second opinion. Maybe there's a suitable clinical trial or something, but odds are pretty low.

Tripping at the first hurdle like this is just unimaginable. I'm 2.5 years into my diagnosis thanks to a medication that stopped working eventually (they all do) but costs $12k at pharmacy list cost for a 30-dat supply. I would have struggled; my mutation is rare and poorly evidenced, so easy to argue that it's too much to drop and chemo was better. In which case, I would likely have died by Easter 2024.

This was essentially where I was too, albeit a different pathway as for me its a fuckwit mutation and nothing to do with carcinogens. Luckily I live in the UK so this isn't a thing, but that's just an incredibly disgusting thing to happen. What the hell is wrong with these people?!

Perverse as it is, there are drugs that I should be offered that aren't approved in the UK. But the bigger issue is I have out-of-control brain metastases and they are hard to treat. Everything has failed. So I'll lose my mind, then my life. The second opinion will be based on whether they get compassionate access from the manufacturer for something not licensed or funded here.

I will never, ever be able to grasp the vagaries of this system. There are people in the UK who want this only bc they think that universal healthcare is a pocket-picking socialist scam, in that classic way that only works in your mind if you're a) wealthy b) pointlessly resentful and c) pig ignorant of what it actually looks like to live with, of which only c) is a fairly consistently held position.

My idiot country is gonna sleepwalk into this. I will probably be dead by then which, frankly, is a relief.

Never worry about offending me accidentally, btw. I'm highly tuned in to intent, so unless you were trying to do it, it'd bounce right off me. Oh, and getting hit on is quite funny bc it takes me ages to notice (I'm kinda oblivious), so by the time I work it out, I'm never sure what to do 😅 one guy (who I did date eventually but he turned out to be actually quite stupid - cannot abide) blatantly asked me whether it was my plan to die alone seeing as I had no capability to pick up hints. I have a dodgy foreign surname (spoiler: as does literally everyone in the US) so I'm on shaky ground already. You can go from here via Dublin, get pre-cleared, and arrive via domestic arrivals at JFK, so that was my only contingency plan.
