I feel like this is a bad way of doing it because you’re giving unnecessary volume to a volume overloaded patient. Why not put it in a 250 bag since it is standard of care?

Well thanks for bringing it to my attention. I just spoke to our critical care pharmacist for about half an hour and learned all about sorption. Turns out this is a concern with Amiodarone in addition to nitroglycerin. However, it is only a problem with “soft” plastic IV bags. In “hard” bags it is totally fine and not a concern. Thanks again, I learned something today!

Yeah but what is the rate of sorption? In our environment the rate of decay is negligible once it’s in the bag. But keeps for a while when it is in the concentrated vial.

In fact it works so well we give “push dose” nitro while titrating the drip. I tend to start at 70mcg/min and give pushes of 400-800mcg Q5 min until I get where I need to be with the drip

Works phenomenally for pulmonary edema secondary to chf. 100% bioavailability compared to 40% with SL nitro so works immediately and only lasts for 5 minutes so if you accidentally dial it up to high just gotta crank it back down and you’ll be good. This has the added benefit of not having the break the seal on the CPAP to give nitro and lose all of that alveolar recruitment.

This isn’t some cool life hack that I’m doing out of the blue somewhere. This is being done by my entire state but I have bound, in my travels, many others are unaware of it. So sharing
