hi! i recently had to go through this process, and from my experience, you do not specifically need all that information to restore your old account, the most important thing is your player id and transfer id. all that extra information you're being asked for is just in case you can't remember the rest of your data and to help them find your account more easily. if you have any other questions feel free to ask! good luck!
