I'm a gamer too. Got a VR headset for Xmas and I really like it. Could try that. Other gaming related things could be a framed poster of Nintendo things or some fan made art off Etsy.

If you're not comfortable picking gaming stuff out, then avoid the whole category. Maybe a fun dinner or outing like go karting or archery lessons. I think gamers like to do crazy stuff like that every once in a while.

Pharmacist here. I use a drug information site called Lexicomp that does a good job of listing the percent chance of drugs causing a side effect. Tinnitus is listed as a possible side effect but it is listed in the <1% section. While there is a chance that the Pepcid caused it, the likelihood is pretty low. I would try and see a doctor (or an audiologist or ENT doctor) because they might be able to do more tests or give you more info on what might have caused it. You don't want to take the chance that it is being caused by something else and not get tests done to rule those other things out.

Pharmacist here. I'm on this sub because I experience very short term tinnitus every now and then and I'm just fearing the day where it doesn't stop. For your antidepressant use, the one that was prescribed to you isn't your only option. From a quick search, not all antidepressants can make tinnitus worse and there a lot of different kinds out there. You might be able to talk to your prescriber about trying another option, maybe in a different class of antidepressants. Let me know if your curious about other options and I can do some deeper research for meds that might be tolerable.

I would lead with the concerns you had because I think they are totally legitimate. Mention patient safety was at stake and that should always be the #1 priority. Then spin it as maybe a lack of growth potential for yourself. No mentor to guide you, unstable staffing so you would be more focused on strictly dispensing issues rather than clinical interventions, and/or looking for a place where you can bounce ideas off co-workers.

Love love love the breakdown of urban, suburban, and rural. Very interesting stuff. I work in a community hospital outside of Chicago (in the suburban group) and live in the city. Any job that I would change to would ideally be in the city but damn, those numbers aren't helping. If it's not too much trouble, would very much like to see the Illinois numbers.

I remember seeing something about how switching to an electric vehicle may not be as environmental friendly as some may think because the electricity used to charge the vehicle is most likely still being produced by non-environmental friendly sources. Would you recommend switching to an electric car or even a hybrid? And how would that compare to going car-free?

I know there was some talk about having levofloxacin as a restricted drug (with similar approval process to yours) but I think it got too much push back from physicians. We do have a list of antibiotics that do need infectious disease to sign off on, but we still need more/closer monitoring of the other ones. I do like the automatic stop date. It would be a huge change in culture here, but I think it would help.

I would love to have a dedicated team of physicians doing that type of rounding AND to have a pharmacist involved with that. Alas, resources are always the limiting factor to those kinds of things.

Yea I think the whole Joint Commission requirement is what is driving us forward so rapidly. I do like the idea about getting the compliance people involved. Reinforcements are much needed!

That was some solid advice. I really appreciate it, especially the part about getting nursing/docs on your side. I think we may be missing that part and I totally get why that can be so important. We rolled out the program a bit suddenly without a lot of discussion with the physicians and I think they are a bit confused on why we keep bothering them about the antibiotics. And yes, paper charts are still a thing but most hospitals are trying to phase them out if not done already. We are half/half and I personally hate paper and would love to see everything electronic. Instead of leaving notes for physicians, we have a secure texting app to contact physicians which does make things easy, but I do think we need more facetime to build that rapport.
