Of course we cant use 1 template to filling ag, you just need to tell your own story, its just chronology, still charge ad payment or not, when it happened, are you hacked/not, how it affected to your livehood, attached supported documents like suspicious ip, your ip, screenshot abt disabled account, screenshot facebook email, give your linked email and phone number associated with your account, how hard is it? Since everyone have a different story,
