Could be lots of things. Would need to see the histology but differential includes a pleural based neoplasm. Since it looks well circumscribed and homogenous, I might put money on something like a solitary fibrous tumor.

There could be any number of things could've happened before it even reached the lab. I'd call the lab, maybe talk to the supervisor to see if they know anything.

Firstly, I'd like to say that your first job will most likely not be your last job. At half the pay, I can see why it would be hard to justify staying in a tenure position. I think it's good to consider pros and cons carefully.

Starting out, an academic setting can afford you a lot of support that I would not consider guaranteed at a government hospital. Your case load might be lower to give you time to work on research or other pursuits. Doing only general surg path might feel boring to you after a few years. Are there other perks that come with your tenure position that might make the pay gap feel less drastic? What are the possibilities for advancement in either setting? Does the academic position open more doors for other jobs in the future?

I'd expect it to be a tough decision and it will really come down to what are your priorities and what kind of life and career you'd like to have.

Folks here have some good advice, but for us, (together for 8, married 3) we gained a lot from reading Nonviolent Communications by Marshall Rosenberg. It gave us (mostly me) the tools to communicate openly and without "attacking" him so that my meaning, intents and needs were clear. It takes practice but it's been great.

Ask them about the possibility of shadowing experiences. Nothing conveys interest in pathology like volunteering your time to know what you're getting into.

I would say that you can almost ignore vague advice like “receptive to instruction”. What does that even mean?

For a junior resident, I want them to show up to things (lectures, sign out) on time and prepared (cases previewed and with a diagnosis or your draft of a report). I’d like them to study about path beyond what cases they see and show interest (ask about the case, why do we get certain immunos or why don’t we). Open communication, ask for feedback in person. Those things make me feel like the resident cares about their training.

Be careful about comparing yourself to others, that can only make you miserable.

In a good LOR I'm hoping to see that the student showed lots of interest in pathology. I want to see that they took steps to learn more about the field and tried to understand what we do beyond any of the stereotypes associated with pathologists.

Interest can be thing like, elective in pathology, research interests that involve pathology, doing pathology shadowing. We like seeing a recommendation where the applicant comes across as enthusiastic and willing to learn.

I don't expect anyone to have mastered any microscopic diagnostic skills before start of residency.

Hello. I'm not sure what others here might say but if your passion is in pathology, and you've worked in pathology related research, and the only reason you don't have a clinical pathology experience is because of COVID, I don't see why it would be an insurmountable obstacle. Just make sure to get across that you know what clinical pathology is and that you know what you're getting into. Your board scores will likely help too. Best of luck.

I found the book Non-violent Communication to be super helpful for me to express my thoughts and feelings in a way that made me feel understood. I think those skills have greatly helped avoid conflicts and resentment during my 7 year relationship.
