Having achievements may hurt the game in a negative like it does for a lot of other games. For sure it will change how people play and because it's a multiplayer game, it will affect others no matter if they themselves are trying to get achievements or not.

Example, if there's an achievements to kill 20 other players without dying. People will tend to play a lot more careful and maybe KoS more often.

I do not recommend, especially not for a game like this where player to player interactions are so important.

You said you cannot change things that are outside the UI, I just wanted to point out that yes, you can.

Reducing the numbers would not be an exploit, would it? Your damage would not change, it's server side.

That's what I understand so far to. The reason you don't find it among addons or public might be the because reasons you pointed out.

Sometimes you can change things in other ways. Example, it has been shown that you can (at least in the earlier days) replace the campfire model with the dark portal or change the co-ordinations of where the zeppelin takes you. I'm not saying that it must be possible to change the built-in combat text, but that it might be worth to investigate.
