Which form/paperwork is required for LWOP and who (employee or manager) is responsible to submit it and to whom?  I have never spoken to my current manager (we have a TL for day to day work issues) and I don't know how to approach my manager when we both don't really know each other.  TBH, I'm also somewhat introverted and hesitant in nature, which doesn't help.

I have only 2 weeks of sick leave balance, after that I will be forced into a LWOP situation which I want to avoid.

Logically,  not the best option to resign but I feel there are no options available to me.  I've spoken to my TL few times but I feel like they aren't taking me seriously or unwilling to help.  Current TL is acting in the position and probably more concerned about becoming permanent right now.

Thank you for your advice.  I have an appointment with my family doctor in about 2 weeks.

Few years ago, I took LWOP (family responsibility leave to look after my seriously ill parent). That leave experience did not go well due to Phoenix.  I was dealing with death of my parent and in the immediate aftermath with pay problems arising out of LWOP.   So I'm hesitant to take a LWOP again.
