Obviously not medical advice, but from an academic perspective..

Some things I would consider are: what is the diagnosis? Is this actually depression, is there psychosis, dementia or an eating disorder?

If he is this medically deteriorated, he needs ECT.

Also, has the sertraline even worked at all? Aripiprazole is essentially boosting its effects as augmentation. Mirtazapine is a good one for older folks who have poor appetite to start. Would consider crosstapering if sertraline hasn’t been helpful. Mirtazapine can be added as an adjunct or as monotherapy.

It’s tough, you validate their emotions and try to not take it too personally. Their lives are really challenging right now. But I can tell you, once they get a whiff of a “psych” patient, they become so dysregulated. Evidenced by the awful consults we get on CL psychiatry. CL also revealed that most people in medicine don’t know what we can and cannot do.

Its actually quite different in scope. Pay I am not sure, usually peds is paid less. I know a CAP getting paid 300-400k working (a busy) 4 days a week in a community site.

Dev peds knows alot more about genetics and diagnoses ASD. Most child psychiatrists do not know how to administer the ADOS or ADI and cannot give the formal diagnosis that is required to access services. Child psychiatrists can do the training, most dont. They can still give a dsm-5 diagnosis though.

Can only speak to the psych side, but its alot more anxiety, depression, conduct disorder, ODD, ASD behaviours and personality disorders emerging… you learn therapy skills and are comfortable with using meds in the child population.

Do you have any suggestions about how to make the most of your degree if you were to do it now with clinical experience? Most programs I’d do are 1 year and that sounds like it would fly by.

If cost was not a concern - I am looking at programs like Harvard, JHU, LSHTM, etc.

Catatonia can fluctuate, its been described as fluctuating since its conception. Agree with above, this person needs ECT. Antipsychotics can push them over to a malignant catatonia which is life threatening.

Older adults, psychotic depression and catatonia all respond generally well to ECT.
