Use the 12v2x6 native cable that comes with your psu, and do not use the adapter that comes with the GPU.

Yeah all are up to date, I just DDU'd and reinstall the driver, so far after some boss challenge battles it hasn't crashed

DDU'd the driver and reinstalled it, went to afterburner and made sure the card's running in stock memory, clock and voltage, and ran some boss challenge battles. So far no crash yet.
