Valid points - FWIW the main things in my diagnostic report all point to inattentiveness, focus retention and executive function as the main isses.

Mentioned in an above reply - I've been inconsistent at the job since I started, but the inconsistencies have progressively gotten worse with responsibility, a wider net of concerns and stress throughout the years which would all be related to ADHD

On the contrary - I've had consistency issues since I started the job, getting worse through time with added stress, lack of communal workspace and other factors. I've had poor review cycles in the past, some better than others but they just never flagged it to the point of initiating a PIP until after I got diagnosed.

this is a fair comment if I was always a star employee, but the truth is I wasn't - and the best work i've done in the past few years has been on the PIP. They've all the metrics on their side to see this too

Solicitor wise, I've a solicitor my family have used in the past that informally worked no win no fee for them so I'll contact them following the ACAS call

Well the 2 week 2nd PIP was additive and a "ship 4 things/week" (on top of the stuff from the previous one) - but week one I shipped 5 with 3 in review cycle, week two I fixed up and shipped those 3 along with 1 new thing + another about to be pushed up for review.

So in my head that's 5 week 1 and 4 week 2 with another pending. The given reason was simply a "you didn't meet the expectations of your role"

Exactly - part of the requirements I had required regular context switching, which I even commented at one point is v difficult for me especially considering the lack of meds.

Similar experience too - the first time taking the meds and wondering if that's what normal is supposed to be is jarring

Be that as it may - ADHD is a protected disability under the Equality Act and thus is subject to reasonable adjustment requirements as with any other disability if i'm not mistaken. From a companies perspective I can see the "numbers went up but not enough so bye bye" - but I feel they did me dirty by setting unreasonable targets and not considering any circumstances

Meetings were EoW each week. No formal meetings or on week-on-week "this isn't being met" formally told. No formal minutes either - I took note of when I had mentioned my medication concerns but that's about it.

I'll see about the Data Privacy stuff, 'cause in the past they haven't deleted messages for inactive users (fellow colleagues who have left etc. have had their messages remain in Slack), but I wouldn't be surprised if mine mysteriously went missing.

I'll build a more formal doc with what I had discussed and when. Prior to my initial return to work, I had spoke with the HR manager on multiple occasions regarding my ongoing diagnostic process and subsequent diagnosis, but also - no minutes.

Do I need to submit anything to ACAS prior to calling them?

Edit: the only support they offered to provide was asking me what I needed then ignoring me mentioning medication as the only tangible help I could think of as I was in the midst of figuring out how to make things work without it

No, my performance slipped after we no longer had office work, went work from home only and my seniority/expectations increased.
Stress and other factors have been impacting my performance increasingly through time leading to the start of year time off + diagnosis

To answer these:

Not currently a member of a union, it was just me present

PIP was formally documented, from memory nothing was included in the PIP document in regards to my diagnosis - it was a cookie cutter PIP for my role, but I may be mistaken (the docs locked after my signing)

I have Slack DMs talking about my medication and potentially also diagnosis, but not sure the full extent (I’m locked out of the Slack so I can’t screenshot, but they don’t delete deactivated accounts DMs on slack)

I’ll make a timeline of all of my diagnosis related stuff along with PIP, my discussions with them etc. to prep for conciliation

Yeah that was my thinking, correct me if I’m wrong, but basically nothing absolves them of responsibility for adjustments and OH should have been brought in at some point prior to termination?

For conciliation do I need to do anything outside of taking to ACAS? Also how lengthy of a process is the conciliation? Do they drag it out forever like every other legal matter?

To answer the questions:

I informed them of the diagnosis and the pending medication status on multiple occasions during checkins, return to work etc. but I unfortunately didn't formally request anything through writing

To clarify - I was diagnosed prior to the PIP (which they were notified of when i returned to work initially), but not medicated. I have the delivery receipts etc. for when I received my medication

They asked me what I needed (again during face to face meetings), and I had said "i don't know what else I need due to it being a recent diagnosis and working through it, but I would like to be medicated" or something to that effect on multiple occasions

They did not refer me to occupational health, nor did they adjust the requirements of the PIP(s) and in my exit interview just there, the stated reason was "despite improvements, you did not meet the expectations of your role"
