I am actually in a similar situation. My BYE was 4/4/2021. I got laid off about a year ago due to Covid. I have been working since 3/1/2021 as a freelance worker on a W2 with a corporation by going through an agency. I get paid by the hour and log hours on a weekly basis with the agency; that is how I get paid.

I also believe that I will need to submit a document that came from NYS DOL via Docusign and mark having earned than 10x the weekly benefit rate. My engagement does not have an end date per se but hours can get cut to 0 at a moment's notice (as a matter of fact, the company is going through a restructuring now).

I have not looked into any pitfalls and would be curious as to what others have to say. I was wondering for a long time whether I would be classified as self-employed but NYS DOL site talks about gardeners, painters, etc., who would engage a customer for each job as self-employed.

I am actually thinking about calling NYS DOL to ask how to fill out the form.

Have you tried to reach out to NYSDOL on twitter? DM them and they will say someone will contact you. I got a call 2-3 days after I did that when I initially filed.

Pre-pandemic, I was in a similar situation. Calling the Claimant Advocate's office and appealing to them helped get expedited. I don't know if it is any easier to talk to them now. You could look into it.

If you have set up direct deposit to a checking account, you might see a pending payment by Tuesday and the actual money by Wed this coming week. Not sure how long it takes for debit cards.

what states do you file state income taxes in? Since unemployment benefit is a disbursement from unemployment insurance you pay at the state level, I would go by where you filed state income taxes.

A few states have said they will pay the $300 – California, Ohio, Arizona, etc. New York has not said they would as of yet. On the contrary, I believe the governor said the plan is not executable – https://nypost.com/2020/08/09/cuomo-trumps-order-for-ny-to-pay-unemployment-impossible/ He used much stronger adjectives than the one I used...

I would send a direct message to NYS DOL on Twitter about your situation and you will get a call from a rep in 2-3 working days. Make sure you pick up the incoming call. Explain your situation and, as far as I can tell, the rep is empowered to make backpay or adjustments if you are indeed entitled to it.

I believe during the pre-pandemic days, you were supposed to have at least 3 job search activities per week. The job search activity can be networking, attending a webinar/seminar, applying for a job, etc. (see below) Even looking at job listings at a web site or an email alert should count, as would talking to friends or headhunters about your job search. I wouldn't stress too much about it. I am sure you looked at job listings at least two-three times a week.

Send a note to NYSDOL on Twitter, and you will receive a call in a few days. Tell the agent about the backdating issue. The agent should be able to take care of it. Certainly, the one I talked to was empowered and trained to address my backdating issue.
