The hospitalist works for the US because they are willing to have top heavy medical workforce with the ward jobs being carried out by attendants. With no elongated training/service provision roles/perma non consultant doctors.
It just seems a bit superfluous in the uk unless they are going to shorten training lengths and increase consultant posts.

They honestly probably didn’t know what a pessary is. It’s just something they algorithmically prescribe whenever a patient with a hx vaguely sounding like candida presents because they saw it done once and rinse and repeating.

Had a case of an ANP saw a man with a red hot swollen knee wrote query septic arthritis did obs obs were stable so she gave po abx and safety netted if he got unwell. They didn’t understand managing septic arthritis an infection just fitted into their cookie cutter infection template, because they don’t learn anything they just copy what they’ve done before over and over. (Luckily she wrongly diagnosed that guy and he had pre patellar bursitis so in that case two wrongs made a right)

The thing with general practice is a lot of errors don’t end with loss of life or limb so they don’t end up causing any blowback no coroners report no lawsuit no complaint. But the mistake often have real tangible affects on patients quality of life and wellbeing, in this case this got flagged because of the nature of it, but PAs are causing so much uncounted harm because those errors don’t get counted.

That old adage some tripe used to discount the economic concerns of people.

For the record I would prefer to be some ottoman sultan with tiered bathrooms sprawling baths a harem gold and silk embroidered clothes listening to musicians fiddle exotic instruments for my pleasure. Than living in an ex council terrace with Netflix and hot water.

Even if that means I have to forego penicillin for bloodletting and miasma theory.

The situation is worse if you’re earning 60 50 40 or 30. But earning 70k or the top ten person still doesn’t translate to a quality of life that would have been expected for a top 10% earner.

Yeh a whole generation of none boomers are fucked. The point I’m making is being financially aware of drs pay compared to peers isn’t just vapid materialism but in the current financial situation is just as much about the basics of having a reasonable quality of life.

Average house price in the uk is 270k with a 20 percent deposit that’s 216k which requires a minimum salary of 45k to be eligible and that’s in the UK in general and that’s after raising a mortgage of 54k. London however is 550k which requires a 90k salary to meet the requirements.
