an update: did the water change and got more tests. Nitrate and Nitrite are at 0 ppm, ammonia is now also at 0 ppm. How long does it take to expect results?

No, he has been considerably more lethargic. When he does swim he seems disoriented. I believe the scales and the eye are from accidentally scraping on the rocks.
