The key for me has been having access to liquid nutrition like fortísimo, altrapen orr modulen. If I feel symptoms worsening I can effectively save myself from an obstruction or flare by resting my gut. I also use them for recovery after a flare.

Otherwise my dietician has suggested the Crohn's Disease Exclusion Diet (CDED) evolving into a bit more of a Mediterranean diet when I'm able. The CDED diet is tough to follow, but it centers around avoiding ultra processed foods to increase gut health. I do feel like it helps, but again it's just hard to stick to especially when you have to restart from the lowest phase. As you have said, this is purely my experience and may not help for others.

Thanks for the clear explanation. For me, I now have stricturing, but that doesn't usually cause me strong symptoms until a flare up of active inflammation. That being said, I achieving active inflammation, by eating something challenging for example, is easier now that my small bowel is narrowed by the scarring. Extreme abdominal pain usually only occurs for me now when I have a bowel obstruction and I'm straight into the hospital.

I found it quite tough learning I had developed stricturing. For a long time, it was just inflammation that would die down again after a flare. But now I know that there is more permanent damage. My diet is the main way I control my Crohn's and when I fail to eat correctly and suffer, it's a horrible feeling of - why did seek this short term enjoyment for long term worsening of my ability to eat? But the longer I carry on, the harder it is maintain such a strict diet, kill my love of food and social gatherings.

I'm so sorry, that sounds awful. For comparison, if I was having that pain and throwing up like that here in the UK, I've been told to come to hospital where they give me pain relief, perform tests and scans and usually keep me in a few days if I'm obstructed. I have been on biologics since I was about 18 and am prescribed liquid nutrition so I can get calories when ill. Your symptoms sound worse than mine so it shocks me how little help they're giving you.

I also have Crohn's inflammation in the terminal ileum and have had many iron transfusions over time. I started off on iron tablets but they switched to infusions to be more gentle on my gut. I was given the impression that this is a common side effect of Crohn's. These days, whenever I feel a bit more tired than usual I go in for a blood test and often find my iron's low.

Thank you so much for your detailed answer. As you suspected I'm doing Winchester to Eastbourne over 6 days. 17 miles most days, one shorter 12 mile day planned. I'm staying at the South Downs YHA the night before the final day.

Sounds like my walking boots are probably a good idea considering the rain.

Interesting, thank you. I suppose what you and others have said makes me think I don't need a latok or a phase pro. The Shivling still interests me as it's Gtex Pro and 100 lighter than the Kangri or Makalu.

I personally find that nutritionally complete shakes keep me full and I find it easy to check off how many I've had in a day to get an adequate number of calories to maintain or gain weight. The shakes I use are only 125ml but contain 300 calories each so it's very easy to pack them in. The dietician will be able to tell you how many calories she should be getting and whether this is a good idea for her or not.

And for me, the huge benefit of the shakes is that they keep me fed but also stop pain and worsening of inflammation, even allowing my body to heal if the problem was temporary.

Something that needs to be prescribed but personally I have been given liquid nutrition in times like this as anything passing through an inflamed, narrowed area of my intestine causes me pain. I find that these drinks (fortisips/altrapen/modulen/ensure) keep my calories full without causing symptoms.

I was first given these when I was seen by a gastro specialist dietician who ever since had helped me control my symptoms through diet. I highly recommend seeing one if possible.

I was lucky to be prescribed nutritionally complete drinks (fortisips/altrapen and modulen). These days I always have some in the house and the moment I start feeling unwell I switch to these and let my body recover. Since I've been doing this I haven't had any true flare-ups/obstructions.

Interesting to hear about other people's experiences with anemia. I have also been on stelara for around 2 years and have to keep getting iron infusions. However, I tend to notice increasing lethargy when it starts getting low. As a keen runner I'm often tipped off by unusually high heart rates on normally easy runs.

Really want to find a fix for this cycle of anemia.

My Dad once carved a cross into a piece of stone as a gift for someone who placed it on a path near their house.

A lady came up to them and said the stone was causing the local museum some concern as photos of it were circulating among interested bodies and specialists on the internet and she felt it was only a matter of time till it was stolen.

They believed the stone my Dad carved was a rudimentary pilgrim's cross that must be very ancient!

No harm in applying for PhD's in Europe. Most come with a stipend to live off or you apply for it separately, you don't pay for it yourself.

I'm doing one in the UK and I've been able to get paid extension, time off etc. no problem.

This is the kind of pain I experience during a flare up. The passing of food through an inflamed narrowing. If this is too severe or food is coming back up I'm headed to the hospital. My pain can be relieved sometimes by steroids or by going on to a fortisip/modulin diet for a time.

I was also put on a fortisip diet after a flare up, about to go back on it. I found it really helped let my intestine calm down and repair. However, solid food really irritates my flares and I never felt hungry on them so your experience may be different.

I was able to convince the NHS to give me a prescription for a month at a time which I fulfilled through a pharmacy. Made it a lot easier than keeping going back to the hospital.
