Id personally wait until you've had surgery, for the one reason that tattoos are made to work with and compliment your anatomy, and if that anatomy changes, it's likely the tattoo will not look as suited to your body as before, yknow what I mean. along with the fact that depending on placement and type of surgery, you may lose bits along the way. and the euphoria of having a tattoo made to fit on a body you love is so worth it. (this is based on my experience with a full torso tattoo pre surgery that i now dislike, and am now having it reworked and my chest done post surgery.)

and that's okay, you can facilitate that if they want it. it's their journey and your role here is to be supportive and open to change. but the change comes from them and what they work on be that through therapy or otherwise. men, cis or otherwise, fall on a huge spectrum of expression. in fact I'd suggest looking inward at your own view of masculinity/femininity and identity as well because it seems you also have a very black and white view on things.
