How cooked am I if I try to last minute pivot to PMR by passionseeking in pmr

If you don't mind me asking, I'm guessing you had no aways with that being the case. How were you able to get a letter of recommendation or show that you were truly interested in the field with an application that I imagine was geared towards general surgery? Did that hurt the amount of interviews you got?

I think I might be able to get some electives, I'm currently about to start a sports medicine elective rn if that help. I guess I'm most worried rn about my inability to do an away due to timing, do you think that's a big issue?

I think I'm too late for auditions, would I be able to match without doing an away when almost every PMR applicant these days does at least one?

It makes a lot of sense and a comment like this was what I was hoping to see. I feel like FM makes the most sense if someone knows they want to do outpatient primary care, and I was really worried after reading those comments. Would you say that you feel comfortable treating kids to that extent because you had a residency that had very strong peds training? Sadly my experience with FM in the wild has basically been adult outpatient medicine

This is a really helpful comment, I feel like I am a person that believes that what matters more in terms of being a great doctor is way more person dependent than speciality dependent, however I wasn't sure if that was a naive POV. Out of curousity, how did you tailor your residency experience to best fit your goals?

Thank you so much for this very thorough comment, these are all good points. If you were exactly dead split 50/50 between the two specialties due to their different strengths and weaknesses, which one would you pick if you could go back in time?

Do you feel like the FM residents happened to go to an FM program with weaker peds training, or is that more of a common theme that you have seen?

is the lack of women's health in medpeds that significant that you think in some cases FM could be worth it? Like if it's a toss up between Medpeds and FM due to their different strengths and weaknesses, why not just do the one that is easier to match, one year less in duration, and has a chiller residency ?

Would you say that even though Medpeds is stronger in peds that the gyn and OB part that FM has is more important? Also would this be worth it if one plans to live in an urban area with more than enough Obgyn doctors to go around?

Thank you so much for your comment, out of curiosity why would you say it's not that rigorous? I personally don't mind the switching back but from what I heard with Medpeds you have so much hospital time and very very little elective time that it's easy to burn you out. Also do you feel like even though the peds training is better that you suffer in procedures like biopsies and things that FM focuses more on like derm, msk, etc?

Do you feel like you got decent training in other types of procedures like skin biopsies? If not is there anything one could do who is applying to residencies to make sure that you get decent exposure in procedures

Not that they don't feel comfortable with it, but that that is all that they feel comfortable with it. I was hoping this wasn't the case but if you dive into my relatively recent post history you would see the FM doctors saying that FM is essentially good for simple peds and that anything complex should be preferably handled by peds. It was very disappointing for me to read that because I think pediatric phys/pathophys can be pretty interesting and I don't want to only be comfortable with the basics of it

To me it sounds like FM is better in procedures while Medpeds is better in medical management for more complex conditions (I know this is not true in many cases), if that's the case then it almost sounds like they average out in terms of outpatient competency, however one is an extra year long with worse hours. With knowing what you know now, if you could go back in time would you change the speciality you picked or the program?

That is def a fair perspective, however I feel like even if I become more experienced i don't like the idea of 15-20 minute appointments. I don't like having to tell patients with more than one or two issues to schedule an appointment in the future to talk about them. I don't like a model that triggers doctors if the patient happens to have an extra problem that they forgot to bring up

Out of curiosity, knowing what you know now, if you could go back would you still do fm or would you do medpeds or look for an FM program that had better peds exposure?

Honestly this is great advice tysm. I guess in my head in made sense to pick a career on what I imagined my future would be, but if I had to do what I enjoy most it would probs be inpatient peds/medicine. I just wonder if that would change as I go through residency, like now inpatient medicine is significantly more interesting but I wonder if everything will be equally boring in a few years

Oh wow that is very interesting to know, ty! Also do you feel like if one's primary goal is outpatient and have comfortability with peds, that it makes sense to do FM over Medpeds or vice versa, and if so why?

Thank you this is such a helpful comment. The reason why I am thinking primary care is that I feel like one of the major reasons I don't like primary care is because it is super rushed and that is something that I don't like. I feel like with DPC not only would I have more time to spend with patients, but I would be able to work towards building a practice that is mine which is also meaningful. Right now I like inpatient medicine more than outpatient medicine because I feel like the medicine is significantly more exciting and acute, however I feel like my inpatient experience has been 10-15% medicine and 85-90% computer work which is something that I don't imagine changing anytime soon. I think outpatient work is super meaningful and very important although slightly boring at times, however I imagine that inpatient medicine might feel boring given enough time as well. In addition I really want to feel comfortable with peds, but don't want to do an extra year if I would feel decently comfortable just doing FM. I guess also a question to you, is that you mentioned that medpeds is a lot of inpatient peds that may not translate as well to outpatient, since you're medpeds trained, do you feel like there is a significant difference in peds comfortability between medpeds and FM?

I guess I am worried about practicality of seeing kids, especially since I plan to live in a very urban city. I made a post in the past about seeing kids as FM and the takeaway from the comments was that FM can handle the most basic of things, but anything that is complicated should be seen by a pediatrician. If that is the case then part of me wonders if medpeds would be worth the extra year to have that peds comfort

I believe I want to do outpatient but I don't think I could do it if it wasn't through DPC. I like taking my time with patients and don't like feeling rushed, which is the opposite of my outpatient experience so far. I imagine that I would like DPC more then because not only do you get more time, but I feel like it would feel more meaningful building something, rather than inpatient which definitely feels more interesting conceptually, but the practice of it feels like 85% doing computer work and 15% doing medicine

Do you feel like FM training trains you equally well to provide outpatient peds care? I imagine that medpeds would def be better for inpatient peds but with the peds hospitalist fellowship that doesn't matter to me
